Empowering Patients

Improving the post-acute care experience for patients living with chronic heart conditions

OBJECTIVE

Explore the relationship between communication, behavior change, and at-home treatment management by elderly patients with chronic heart conditions. Looking specifically at the following question:

How might we help elderly patients with chronic cardiovascular disease better understand and manage their treatment plan at home to reduce the likelihood of readmission?

Visualizing Pain

An exploration of material and medium as a means of communication

OBJECTIVE

Explore our relationship with pain and the ways different materials and mediums can be used to help individuals with chronic pain contextualize their pain to others.
